New Patient Registration

registrationIf you would like to become a patient with Southdene Medical Centre, please contact reception and make an appointment with our Health Care Assistant  for a New patient Check. When you attend this appointment, please bring with you your Registration form, Health Questionnaire, Photo ID and proof of address, if available. During this appointment you will be registered with the practice and receive a simple health check and we will take details of your medical history.

If you are on any medication, please make an appointment with the doctor. This will assist us while we are waiting for your medical records to arrive.
